I had so much to say for this one. And then I forgot it all XD;

Title comes from Metric - Monster Hospital. The writing says "The lunatics have taken over the asylum", taken from a song by the same name by Collide. Both were the inspirations.

She's supposed to have different colored eyes. The lunatic in the back is chewing someone elses arm and there is a pen stuck through the cheek of the closest lunatic.
